ID Card Declaration Forms

This service provided by the Information and eGovernment Authority allows individuals to download and fill the declaration forms that are used in some requests related to issuing identity cards, update ID and civil record information for individuals.

Service Conditions

  • A declaration of accommodation form: used for the approving of the address owner to use his residential address for a family member or relative.
  • Declaration of Transferring Head of Household Relationship form: Used for the husband's consent to register his wife as the head of the household.
  • Declaration of Custody Agreement Between the Two Parties: It is used for the signatures of both parties (the divorced parents) for children under 21 years old, to registering the second party mentioned in this agreement as a head of family for the child, and to add the child's residence address with the second party.
  • Declaration of accommodation transfer: used for choosing residential address in specific cases for example (children from 21 years and above have divorced parents).
