Required Attachments
- Copy of audited annual financial statement
- GOSI statement
- Copy of audited report for the actual consumption of spare parts for the last two years (if applicable)

- Search by entering the CR or Industrial License number without entering the branch number
- Sselect the required branch
- Fill in the application and attach the required document
