Add Activity for providing shisha (smoking)
Add a sub-activity to the previously licensed health certificate
Service Conditions
- A valid commercial register subject to health control.
- Fill out the registration form.
Required Attachments
- A copy of the lease contract or a copy of the title deed
- Address identification card (municipality) or electricity bill.
- An engineering chart of areas (in meters) with mention of the work areas in the facility, provided that they are drawn with dry ink or printed electronically.
- A layout of the shop showing the smoking area, the non-smoking area, and the Sheesh preparation room, provided that the length, width and height measurements for each area are recorded, bearing in mind that the area of the smoking area should not exceed 50% of the area of the closed sitting rooms, (serve Sheesha)
- A copy of the commercial register.
- Registration unit form.
- Applicant CPR.
