Issuance of temporary health certificate (kiosk)
certificate to practice commercial activities under health surveillance
Service Conditions
- A valid commercial register subject to health control.
- Or a supply certificate.
- Fill out the follow-up form for licenses and temporary certificates.
Required Attachments
- Request letter for health inspection.
- The previous temporary health certificate (for renewal).
- CPR card (for the owner or authorized person).
- A valid rental contract.
- Geometric diagram with areas clarification.
- A valid commercial register.
- Supply certificate.
- Fill out the form to request the temporary certificate.
