Service Conditions

  • Obtained final approval from the Ministry of Health.
  • The final approval does not exceed two years.
  • Issuance of commercial registration (valid).
  • Fill out the registration form.

  • Copy of the commercial registration certificate/CR. 
  • Registration unit Form.
