Change Activity
Modify a sub-activity to the previously licensed health certificate
Service Conditions
- A valid commercial register subject to health control
- Fill out the registration form.
Required Attachments
- A copy of the lease contract or a copy of the title deed
- Address identification card (municipality) or electricity bill.
- An engineering chart of areas (in meters) with mention of the work areas in the facility, provided that they are drawn with dry ink or printed electronically.
- A copy of the commercial register.
- Registration unit form.
- Applicant CPR.
