Required Attachments
- Fill out the accident report request form
- ID card of the applicant
- A copy of the electricity bill or any document proving ownership of the house or property
- The lease contract, in the event that the applicant is a tenant
- Small Business Registry

- Exempt from fees according to Ministerial Edict (27) for the year 2020

Service Processes
- Fill out the accident report request form
- Review the General Directorate of Civil Defense and apply for a fire report
- Fill out the form for requesting a civil defense report on the accident/fire
- Show ID card (valid)
- A copy of the commercial register in case it is a small establishment
