Late notification reports
Issuing reports on delayed fire reports that were initiated long after the accident ended
Required Attachments
- Fill out the accident report request form
- ID card of the applicant
- Authorization letter from the owner (person/company) of the applicant in case he does not appear in person to receive the report
- Small Business Registry
- The lease contract in the event that the applicant is a tenant
- Electricity bill or property ownership document
Legal Regulations
- Exempt from fees according to Ministerial Edict (27) for the year 2020

Service Processes
- Refer to the security police station responsible for the area in which the fire or accident occurred to make a report regarding the accident to obtain despite the report
- Review the General Directorate of Civil Defense and apply for a fire report
- Fill out the form for requesting a civil defense report on the accident/fire
