Replace a lost or damaged report (all types of reports)

This service involves the issuance of a replacement copy of the original report issued by the General Directorate of Civil Defence, submitted by the owner of the establishment/vehicle

  • Completed accident report request form
  • Applicant’s Identity Card
  • Copy of the previous report (if any)

Service Processes

  • Fill out the accident report request form
  • Present the applicant’s Identity Card
  • The new report is delivered to the individual who lost or damaged the original report. Alternatively, the new report can be delivered to an authorized representative, provided they acquire an authorization letter signed by the individual
