National Genome Program forms

From government's perspective to raise the quality of health services and prevent diseases and take into account the latest innovative scientific methods, the idea of ​​establishing a specialized center for genetic analysis was born. The Ministry of Health has opened the National Genome Center to provide better healthy life opportunities for future generations, improve treatment opportunities, raise the quality of health services and prevent diseases, that is through analyzing databases, which will greatly contribute to improving the diagnosis and early detection of diseases, and may also contribute to the development of effective medicines to treat them. This service has been developed to facilitates the process of participating in or withdrawing from the National Genome Program, in addition to the possibility of requesting results (if available); It allows to carry out all these operations electronically through three types of forms: Participation form / Withdrawal form / Request results form.
