Service application form.
The original passport of workers and a copy of it.
A copy of the guarantors ID card, and a copy of the commercial registration.
A copy of the applicants card or legal representative.

Note:

In the case of a local transfer, an official letter is attached from the new guarantor or the institution to which the transfer is made to.
