Optional Insurance Registration - Continue (Resolution No. 1 of 94)

Submit a request electronically to the customer service centre for the continuation of optional insurance under Ministerial Resolution No. (1) of 1994. In-person attendance is not required to process the request

  • Optional Insurance form
  • A copy of the Identity Card from both sides (front and back)
  • A copy of the individual’s passport
