Death Certificate Request
This service, provided by the Information & eGovernment Authority (iGA), allows authorized individuals to issue a death certificate for a person who has passed away within the Kingdom of Bahrain.
Service Conditions
- Hospital's original medical report detailing the cause of death
- ID of the deceased (ID card or passport)
- ID of the individual reporting the death (ID card or passport)
- Filling out the Death Notification Form
- Deaths must be reported within 72 hours as per Article (12) of the Deaths Law which requires reporting deaths in Bahrain within 72 hours and those outside the Kingdom within 60 days
Required Attachments
- Fill out the death notification form
- Identity of the deceased
- Identity of the deceased’s family
- Notification of cause of death / death certificate certified by the country where the death occurred (death outside Bahrain) for Bahrainis only

- An organizational resolution regarding the procedures of the Births and Deaths Department.

Fees
- 0.4 BD : The certificate issuance fee if notification is made after 72 hours and up to three months from the date of death
- 0.8 BD : The fee for issuing a death certificate if the notification is made after 3 months and up to twelve months from the date of death
- Free : For first 72 hours

- In-person attendance at the birth and death registration unit
- Print the certificate and affidavit of death to whom it may concern
- Filling the death certificate application form and submitting the required documents.
- Pay the required fees and receive a replacement for the death certificate

- Electronic payment of service fees
- Receive the death certificate through Bahrain Post
