Replacement/Extract of Death Certificate
This service from the Information & eGovernment Authority (iGA) enables authorized individuals to issue a replacement or extract death certificate for deceased individuals who already possess a death certificate issued by the country.
Service Conditions
- ID of the deceased (ID card or passport)
- ID of the person reporting the death (ID card or passport)
- Completed form for a replacement/duplicate death certificate (if applying in-person)
- The applicant must be a first-degree relative (provide proof of relationship)
- Legal inheritance document if no direct relationship exists
- Official general or specific power of attorney for the death certificate
- Proof of legitimate interest for in-person requests to issue the death certificate
Required Attachments
- Fill out a request form to replace a lost death certificate
- ID card(s) or passport(s) of the deceased’s family member(s), and a copy of the respective document
- The deceased’s passport and copy, plus an ID card, if any, and a copy
- For the authorized person: Proof of authorization with an ID card, plus an additional copy

Fees
- 1 BD : A fee of one dinar is required to obtain a copy of the birth or death certificate if the submitted application states the date of birth/death in terms of the day, month and year, and the fee is doubled if the application solely states the year of the occurrence

- Receive the death certificate replacement through Bahrain Post
- In-person attendance at birth and death registration unit
- Fill out the replacement request form and attach the required documents
- Pay the required fees and collect a replacement for the death certificate
