Issuing a Replacement for a Lost Birth Certificate
Request a replacement of the birth certificate and pay the fees online.
Service Conditions
- Completed birth certificate replacement/extract of birth certificate form
- Copy of the applicant's and parents' ID cards or passports
Required Attachments
- Fill out the application form to replace the lost birth certificate
- A copy of the cards or passports of the applicant, father and mother
- A copy of the parents’ marriage contract, if any
- A copy of the birth certificate, if any
- Authorization from the applicant in the event of his absence in person
Legal Regulations
- Law No. (7) of 2019 regulating the registration of births and deaths
- A draft resolution on the issuance of the executive by laws
- A resolution regulating procedures for registering and issuing birth and death certificates
- An organizational resolution regarding the procedures of the Births and Deaths Department.
Fees
- 1 BD : A fee of one dinar is required to obtain an extract of the birth or death certificate if the submitted application states the date of the occurrence in terms of day, month and year, and the fee is doubled if the application solely states the year of the occurrence
