- Thermal Insulation Product Registration Form
- Copy of the commercial registration certificate
- Authorization from the manufacturer, if the applicant is not the manufacturer, confirming that the applicant is allowed to have the required approvals from the relevant authorities
- Pictures, specifications, and catalogs/brochures of the product
- Test report from an independent an laboratory documenting the reflection coefficient and radiation coefficient
- Accreditation in the form of an ISO-17205 test report from an independent laboratory for the reflection coefficient and radiation coefficient
