Filling the death notification form
Medical notification for the death cause certified by the treating doctor, hospital, or forensic doctor.
The identity card or passport of the deceased and applicant

Death outside the Kingdom of Bahrain:

Filling the death notification form
The original death certificate attested by the embassy, consulate, or diplomatic representation office of the Kingdom of Bahrain, or with the apostille stamp
The identity card or passport of the deceased and applicant
