  1. Fire Report application request form
  2. Police station case number
  3. Copy of applicant’s valid Identity Card
  4. Official fire request letter submitted by the company mentioning the authorized person to receive the report
  5. Copy of the Commercial Registration
  6. In case of rentals or leasing, a copy of the lease contract is required
